We are here to provide your child with the best health care possible. Our goal is to safely return them to competition as soon as they are ready. We encourage you to go through the Certified Athletic Trainer before seeing a doctor to possibly save you time and money. Our Team Physician, Dr. Steven Thornton is who we try to schedule appointments with when your student-athlete suffers an injury. Please feel free to contact the athletic trainer if you ever have questions or concerns.
FORMS:
All athletes must have a UIL Physical before entering their 7th, 9th, and 11th grades to participate in Athletics, Cheer, Drill Team, and Band. They must also have an Emergency Release Packet which includes; the Release Form, UIL Acknowledgement of Rules, Emergency Information form, CISD Insurance Waiver, UIL Illegal Steroid Use form, and the Mandatory Drug Testing form. All of these forms must be turned in prior to participation annually. You can access the forms by clicking on each link below and printing them off. These forms must be turned in to the Athletic Trainer’s office. DO NOT turn these in to the front office, because they are not responsible for keeping up with them.

Student Insurance
The school offers a student insurance that will serve as a secondary insurance to supplement your existing coverage. An insurance brochure will be sent home at the beginning of each school year. This relatively inexpensive insurance could potentially save you thousands of dollars should a major injury occur.
